any formal template for minutes taking?

Ask the former chairperson how they took the minute notes and that usually the best way.

Microsoft Word Perfect 12, Has templates for minute taking. So I am sure there must be other software that does the same.
Try going to www.cnet.com
They have a lot of software downloads that are free. You will be able to check out the different software brands before you buy.
But I always use Microsoft word perfect 12. I have used it and previous versions for years. And have been very happy with them.

Normally, if there is one present, it would be the secretary, but there are many meetings where minutes are taken by one of the persons participating and designated there and then to do so. In that case it is either "the minutes recorder","the minutes taker" or, just as you defined, "the person who took the minutes".
