We are not married and the baby will have his last name. Does he need to sign? Do I need to provide his SIN #? How does this work?

no he doesn't need to sign- I went through this with my son- they do not need to sign. Also, you can just say you don't know who the father is- it saves a LOT of legal trouble. I didn't end up putting my sons father on the birth cert. because he's a dufus, but I did still end up taking him to court for support and for him to sign over rights to my son. So, no the father does not need to sign anything. My son has my last name though...
But since you say the baby will have his last name, I am not 100% sure- I think in that case the father does have to sign a birth registry...
I don't know the situation, but if the father is not going to be in the childs life, I think personally speaking, it is easier not to name them and to give the baby the same last name as you..... ask a paralegal about it, they offer free consultations. Or ask a family services worker.

If you are putting him on there then he does need to sign. IF you do not want anyone to sign then no you do not need a signature. Yes you do need the sin number for him if he is to sign.

No, only 1 parent needs to it could be the mother or the father.
