A woman's body will produce as much milk as the baby needs. As the baby gradually cuts down on breastfeeding as he/she gets older, so will the milk supply decrease.

The size of the breasts are mostly determined by how much fatty tissue are on them, and has nothing to do with milk production. The only exception is that a woman who is extrememly flat may have some kind of hormonal imbalance which caused her never to develop in the first place, and therefore she may not have the right hormones for adequate milk production. However, certainly she would know that about herself prior to giving birth.

Except in extreme cases, I would say no. I have heard of very small very flat chested women sometimes being unable to produce quite enough milk, but I'm not sure if that was the root problem.
I am very large chested and I had the lactation consultant tell me I had very good "storage capacity." So maybe it makes a difference in how much is stored, but not how much is produced.

Absolutely Not...that is a myth. I know it may seem correct. But I read up on it once. Women produce an average of 4-6oz. of milk per breast per feeding. Here is an interesting article on nursing below.
It's mostly about believing in yourself, relaxing, and making sure the baby latches on correctly also feed for 15-30 mins per side from the start to ensure proper supply. I know the "believing in yourself" sounds silly...but it's true. If you are stressed about it your milk won't let down. Think about it, women have been feeding babies forever. We are built to sustain our children.

no, the glands beneath the breast are all relatively the same size and it is hormones that produce the milk, it has nothing to do with the actual cup size

No, not at all. Breast size is all about fatty tissue, which has nothing to do with breastfeeding. The plumbing inside the breast is more important to breastfeeding. That, and nursing frequently enough since breastfeeding is all about supply and demand.
