I was about 19 and I was talking to two older ladies and we were talking about driving a car. I said I was an "exotic driver".
I meant to say I was an "Erratic Driver". They both just smiled at me and kept talking.

I tried using the word quarrel. I know why it is not used often. It sounds retarded, like you are trying to say squirrel, but got punched or something.

I went to the pharmacy to pick up my "prescription" and accidentally asked for my subscription. Boy did I feel stupid.

my husband woke me up in the morning and told me to get up. i was still half asleep and started talking to him in my native language (which he doesn't speak). he told me he doesn't understand me and i got angry cause i thought he was making fun of me (i didn't realize i was talking in another language). this went on for a good 3 minutes ... i was so angry at him (i am kinda grumpy when i wake up the morning).
when i finally realized what was going on i felt so stupid.

other situation: i translated a figure of speech into american and it accidentally turned into a racial slur that i didn't know about.
that was probably the worst one.

Well, not exactly, but I did forget how to spell "of" once. "Ov" and "uv" didn't look right, but I just couldn'tremeber because it was so simple a word.
