I am not sure about the php part, but http://250free.com lets you create a site. Has a good help forum for the site and you can store up to 250mb free. Bandwidth is large and resets every month. I am not overly savvy on doing stuff myself, and have had few problems. You can check out mine at http://ladyemberrose.250free.com and see what you think. You can email me from there with my link and I can try to help you further. I do know some sites but too numerous to name them all here. Only a few things I don't have on my site like music and video things.

with latest version of frontpage, i believe you can do that. It have such the abilities from scratch, so you'd just have to design.
But i think theres no provider which allows php for free, unless with a pay.(you can try to search it from the web). If you want to have it yourself, you must have your own server and register it as your DNS so that you have www.yourname.com. For how to register a DNS, please refer to someone , i havent tried this before.
