The sixth sense comes to mind.. but I wouldn't say it was the most shocking in cinema history. I'd have to really think about that one.

Maybe not the most shocking twist in all of cinema history but the last one I saw that completely shocked me was Derailed, with Jennifer Aniston.
I agree with the people that said Saw, the end really did blow you away. Same goes for The Sixth Sense.

I'm not sure I'm getting the queastion correctly, and I couldn't say about cinema history because that's a bit too large, but among the movies I've seen I'm thinking Fight Club probably surprised me the most.

The Departed. Nobody expected the twists at the end of that movie.

Something that totally messed with my mind?
You know the end of that one part in Creepshow, where the water zombies of the wife and her lover grab Leslie Nielsen and bury him where he's going to drown? That freaked me out.
Mostly because I knew Leslie Nielsen from goofy movies, and this particular role was all serious and dark and creepy and he did really great in that part, that homicidal maniac part.

The original Saw-
Fight club was close...but a bit predictable, but the ending to Saw had everyone in the movie theatre gasping.

Psycho. It's known so well now, but at the time... It starts out with a young woman with a problem lover who steals a bunch of money and takes off to meet him. It seems like a crime story/love story, but then she takes a shower...
