Jedi Knight Stass Allie was the one killed on Saleucami while on a speederbike patrol when Order 66 was given. She wasn't a Twi'lek she was a dark skinned near-human species.

The only Twi'lek shown being killed during Order 66 was the blue skinned Jedi Knight Aayla Secura on Felucia, and she was on foot when the ambush occured.

Other Jedi shown killed during Order 66 were:

Ki-Adi-Mundi, he was killed on the icy planet of Mygeeto.

Ploo Koon, he is shot down by his wingman on Cato Neimoidia.

Zett Jukassa, he is the teen Padawan killed in front of Bail Organa on the landing pad to the Jedi Temple.

**Luminara Unduli, she is shown on Kashyyyk helping Yoda coordinate defenses with the Wookiees and clone troopers, it is assumed the clones close to her were successful in completing Order 66 since only Yoda survives.

**Shaak Ti is killed by General Grievous in a deleted scene, but in the final release she appears as a hologram during one of the briefings, her death is never shown in the final version of the film, but it is assumed she did not survive Order 66.

**Mace Windu, Kit Fisto, Agen Kolar, and Saesee Tiin are the four Jedi Masters killed in the attempt to arrest Palpatine.

I believe you're referring to Aayla Secura. According to the official Star Wars databank, she was a general in the Republic, but her manner of death is not precisely what you described: "As General Secura approached the opposing Separatist forces, Chancellor Palpatine activated Order 66 on Coruscant, and contacted Aayla's clone officer, Commander Bly, via hologram. The executive command identified the Jedi as traitors to the Republic. Bly loyally carried out the order, raising his blaster rifle against his Jedi general.

In a moment of distraction, as a native Felucian bird took wing and drew Aayla's attentions skyward, Bly and his troops opened fire, riddling Secura's body with blaster bolts and killing her."

Although this detail doesn't match, she's the only Twi'lek Jedi with a name, and she certainly would have been of interest to the forces loyal to Palpatine.
