My name in H.Sc certificate is Akhilesh. want to write it as Akhilesh Nautiyal in passport. What to do?

I want to make passport, in which I want to write my full name. In my H.Sc certificate I have only Akhilesh. Nautiyal is my surname. I am doing Ph.D and in my Phd thesis I want to write my full name as Akhilesh Nautiyal. I live in Gujarat, India. Please tell me what to do?

Write your name as indicated on the passport because all your documents bearing yor name will be matched to the passport if you will work or travel.
