It doesn't matter if he's skinny and kind--he is your "best friend", therefore gift cards and impersonal gifts of any kind are OUT!
How about a punching bag? He can be abusive to it and work off a few pounds :)

if your friend likes drinking beer get him this kick *** belt buckle so he can pop the bottle cap open,I'm pretty sure it's sold in Anchor Blue and if not try looking in the rest of the mall.
