If a student gets good grades, but looks like they don't care for themselves, looks sad, tired with dark contours under eyes, agitated, stutters, and even trembles sometimes, and comes up with bizarre ideas, forgets things and is confused...what does this mean? Should it be taken seriously or because they get good grades it means they're fine..?

Of course it should be taken seriously. Talk to the person concerned and say that having noticed the changes, you would like to offer your assistance.

For one thing you're smart, that's good. But whatever you're going through or went through is making your life condition very low, that's not good. You don't want to take it too seriously cuz then you'll be more confused. Best thing you can do is to try new things, always keep ya head up, and have faith no matter what.

It doesn't sound like he's fine. People can be intelligent and physically or emotionally ill at the same time.
