The first question I ask myself is, "What if ......" you can fill in anything you want for the blank. Usually something I have seen or heard triggers it for me. What if a young boy had a dream about building a receiver that could convert animal language into language that he could understand? That's one that came to me while listening to whales on a sonar hookup at the acquarium. What would he do with such a device? And of course, you would have to have some conflict. Parents, first of all, who reprimand him for wasting time and money on such nonsense. And then you could throw in the evil industrialist who wants to steal the device and sell it to hunters so they could hear their prey and the animals would have no chance to escape. Or, or, or.
Anyway, I usually try to set up a situation, and my characters grow out of that, and are created as they are needed.
Does that help at all?

Well those are the questions. Of course they spin more questions for you as you ask them.
Who: Character list, Names, Ages, etc etc
What: What's going on in this story, what is the major plot/idea for your book.. etc etc
When: What is the era you are writing this book in?
I have found writing a outline very helpful before writing anything. Perhaps you can try that. Best of luck with your writing.

I ask myself who I'm writing for. I usually try to pick a specific person to tell my story to; it's intimidating writing for humanity in general, and focussing down on the possible reactions of one person really helps. It also helps me to sort out which of my ideas are better.

As stupid and hare-brained as it sounds, I just start typing. Somehow a little of my life or personality comes out in my writing, but it works out great, and it is painless.
I do not ask myself questions.
Writing is an act of discovery!
