You have to see a psychologist to have it tested 'officially'

However, there are some IQ test duplicates you can take on the internet for free! Just be honest when taking them and don't cheat and you'll get a valid score.

First one is fairly culture saturated, but tests a wide range of intelligences. The second test is considered culture fair. Both have a maximum score in the 140's, so if you score over 140 you may require a higher range test to truely know your IQ.

You can get one at a book store. Libraries may also have them.
This will give you a pretty good idea of your IQ, IF you don't cheat.
If you need a witness for your score, consider hiring a notary or proctor, or perhaps a teacher to oversee your test taking.
Depends on what you need the test for: your own curiosity, or as part of a resume or other-impressing tool?

Ask a psychologist at your school, and I will point out that if you are extremely gifted, than someone at your school will ask you to take one. You can take one through Mensa as well.
