It comes out of the top you are wearing, ie bra, shirt, bathing suit, etc.

It means you've had a failure of the containment vessel. :-)

Often, this phrase refers to what happens when the shirt you're wearing loses a button (or a button comes open). It can also mean that the top has slipped to one side, allowing your breast to be uncovered. (This is also sometimes referred to as a "nip slip.")

It can also mean what happens if the undergarment you're wearing doesn't fit properly and your breast is squeezed out over the top of your bra, even though it stays inside your top. Sometimes this is a gradual process; sometimes it's sudden. Sometimes it only pops halfway out (you know the look, where there's a diagonal crease up the middle); sometimes (but rarely) it comes all the way out.

And to show that the same phrase can have two very different meanings, consider this sentence: "When her boobs popped out, his eyes popped out."

Example: Your bathing suit top gets out of place and your boob is showing unexpectedly, to your surprise you quickly fix your top back into place. You could then say oops my boob popped out. Funny but embarrassing.
