my husband died and i want to change my name back to my maiden name. like in divorced you have paperwork you have to provide social security and dmv. what will i need to change my name back without paperwork or what kind of paperwork do i need .

Go to your county court house and see the records clerk. They will be able to help you...you still have to fill uout paper work but it doesn't matter as to why you are doing it...you do not need a reason. Enjoy! :)

I don't think that it should be a part of the divorce i think that if you are getting a divorce you would want to get rid his name too Besides kids why keep something as a constant reminder of him? There was a lady at my job that got divorced and she didn't change her information on her paperwork at our office but her name was legally changed. when i called her name she got mad at me and said DO NOT CALL ME BY THAT NAME, THAT IS NOT MY NAME. i looked at her and the paperwork and just shook my head i didn't understand at first but now i do. In the end ladies it is ultamately your decision on what name you want. I have been married a year and a half and we have 4 kids our oldest has my last name and our other 3 have his, i still have my maiden name too. i just dont know if i want to change it and what i want it to be exactly.

go to the court house and apply for a legal name change
In Wisconsin, the name change request has to be published in the area newspaper for three weeks. I think that is so if you have any debts, people will know you are about to make a name change.

Anyway, after the three weeks, you have a court date and the judge asks if its correct that you want to change your name.

Thats it. I know the procedure - my wife, still married wife changed her name. There doesnt need to be a marrage, divorce or death.

Ok I know in my state you have to do it through social security. You take your birth certificate and his death certificate to the local office fill out a form of why you are changing your name and with in 6-8 weeks you get a new social security card and take it with you to DMV.

I think anyone can change their name to anything they want. All you have to do is fill out a change of name form with the Social Security administration. I don't think you even need a reason.

Petition the local court for a legal change of name. Once you have the order signed, you should be able to change your name on all your other documents.

I don't know why you would want to do that though.

You go to your local county courthouse and apply to change your name. They of course charge for this service. Call the courthouse to check on fees and anything you will need to take with you when you get it changed.

All you need to do is file for a name change. Yes you have to show identification

its hell just to change a name back. all you would need is your amrriage certificate and his death certificate. bring it to the social security office. you may want to bring the social security cards too just to be safe, but definitely the death certificate.
