I've been doing a lot of travel lately for business reasons, and I gotta ask... what happened to terminal 4 at OHare? I've had flights leave from Terminal 3 and flights leave from Terminal 5, but as far as I can tell, Terminal 4 just isn't. What happened? Do I chalk it up to the "New Math"?

they don't use terminal 4 anymore. when they built the new international terminal, they named it terminal 5 in case they ever decide to remodel or expand. that way, all the numbers still go in order.

According to Wikipedia, and my own personal experience, Terminal 4 was the interim international terminal, and it was located at the ground floor of the central parking garage. International is now in Terminal 5, and they haven't used the Terminal 4 designation since. Guess they could've moved the designation to the new International terminal, but they didn't.
