Explains a great deal about the process of diffusion and how it effects concentration levels. And the quick answer to your question of course would be that diffusion usually lowers concentration levels on one side of the equation, while raising the concentration levels on another side, until an equilibrium concentration is reached.

due to diffusion, substances spread out until a dynamic equillibrium is reached.
this is when the concentrations are the same inside and out.
