There are some very cool vintage looking journals that she may like. Check out barnesnoble.com.

find out her favourite theme, buy a symbolic gift, or make something yourself
the trick is to be thoughtful, and tho show you truly had her in mind: that's 90% of success

my father wanted an expensive car, there was no way I could buy it for him - one year we bought a small puzzle with the picture of that model, another year a keychain, and a small model of that car, and such

you can find pictures of her favourite artist(s), find some biography details, their quotes (or her favorite quotes), and make a collage of his arts and pictures, or a compilation, or a memo album/scrapbook, decorate it etc.
You can also compile a scrapbook (and find a relevant pics), made solely of her favourite quotes, theme: art, music, or quotes of artists and musicians. If she is a journal writer, you could also turn it into a journal, simply by leaving one side empty.

You can find a bunch of quotes and info on the internet, and sizzix and wilde-ideas, and local scrapbook stores offer lot of materials. If you think it's not your thing, you can ask your girlfriend (or a girl friend, whatever works for you) to help you out with a surprise (also a nice excuse for a non-date ;D).

Idea: if you were her, would you appreciate this? If you put your heart in it to design, fill it with appropriate content, decorate it...
I am sure it might be even worth more than if you just went into the store and cashout for a gift. I am an artistic soul myself, and I know I would, especially since I also know how much effort I put into making something for somebody.

If she wears jewelry, you can usually get some pretty cheap stuff at certain stores at the mall, other than that take her shopping with you, if she sees something she likes, go back later without her and buy it for her, that's what i usually do.

If you can afford it, buy her a "coffee table" book of gorgeous record album covers. If not, buy her a CD of music that she will enjoy--maybe a Christmas album by one of her favorite artists.

Something that she wants. I suggest you ask that is what most people do.
