Yes, it all depends on how the government is set up. President and dictator are just words, it really depends on who is making the decisions for the country. Not all countries with a president have a truely balanced government nor are all of them democratically elected.

In modern times virtually all dictators have been "elected". Those are the so called one party states. They generally require that all qualified individuals vote, although there is usually only one candidate on the ballot.
