I always hear it referred to, but never in detail. I remember an old JSA cover where "Batman speaks from beyond the grave." Wikipedia says, "Wayne retires from the position to become police commissioner, a position he occupies until he is killed during one final adventure as Batman. " Again, no reference to documentation.

UPDATE:
Bill Jensen, a man convicted of murder during Bruce Wayne’s tenure as police commissioner, escapes from prison. He soon reappears atop Gotham’s Trade Tower, now possessing mysterious new super-powers strong enough to stagger even Dr. Fate and Green Lantern. He threatens to lay waste to Gotham City unless Bruce Wayne surrenders to him. PL/JSt/BL
Notes: The two-part story in Adventure Comics #461-#462 was originally intended to run in the never-published All-Star Comics #75 (10-11/78). The text of the story said that Selina Kyle died “last summer,” despite the fact that DC Super-Stars #17 (11-12/77) specifically showed her death as occurring in late summer 1976. The later Last Days of the JSA (1986) said that Batman died “in 1979 -- several years after the death of his late wife.” ADV 461 1-2/79
Bruce Wayne is forced to become Batman a final time to battle Bill Jensen. During their fight, Jensen’s energy blasts rip away a portion of Batman’s mask. When Jensen sees who his foe really is, his power runs out of control in a final burst of rage, killing both men. Batman is laid to rest next to the graves of his wife and parents. His funeral is attended by Helena Wayne, Dick Grayson, Alfred Beagle, and the Justice Society. At the funeral, a grieving Helena Wayne persuades Dick Grayson not to take up Batman’s mantle, preferring to let her father’s memory and legacy rest in peace. PL/JSt/BL ADV 462 3-4/79
The Justice Society tracks down Frederic Vaux, the sorcerer who gave Bill Jensen his powers, and avenge Batman’s death. To safeguard Dick Grayson and Helena Wayne’s secret identities, Dr. Fate casts a spell to ensure that everyone who did not previously know Batman’s true identity believes that Bruce Wayne and Batman died separate deaths and were buried in separate graves. there is not any one e book which will clarify each thing by way of finished elsewords memories, or diverse earths component (for tale/rationalization of diverse earth especially study disaster of limitless Earths). The DC historic past is slightly screwy, and that they've made tries to restoration/streamline/clarify it countless situations. The DC comics Encyclopedia delivers you the run down of each character (some irrelevant characters yet you may %. & pick what to study). it delivers you the fundamentals on the super ones, sufficient to bounce in and initiate analyzing. Golden & silver are in simple terms referances to situations wherein comics have been printed. they even have not got any relevance to the characters or memories. For sturdy collections of the previous memories look on the line that DC positioned out spoke of as the "exhibit delivers". they are sturdy black & white collections of the previous memories at a good cost. they have them for many of the suitable titles of the time (Batman, Superman, ask your self woman, JLA, eco-friendly Lantern, Flash, Atom, eco-friendly Arrow, brave & the ambitious, and a few 'minor' titles like Jonah Hex, Batgirl, Metamorpho, between others). sturdy readin'.

I thought it was established in the origin of the Huntress that the Joker planted a bomb under the staging where Helena Wayne's parents were due to speak in public. Try Toonopedia, they have lots more specific information.
