There are too many to name but the sources my company has come across the most include above and underground storage tanks, older industrial facilities that used improper landfill techniques, and dry cleaners. The VT link goes into a lot more detail.

There are certainly a lot of sources of groundwater pollution. They include (but are not limited to):

(1) Contaminants in rain, snow, and dry atmospheric fallout

(2) Municipal sources:
air pollution
municipal waste land spreading
salt for de-icing streets
streets & parking lots
landfills
graveyards
wells for disposal of liquid wastes
leaky sewer lines

(3) Industrial sources:
air pollution
chemicals: storage & spills
fuels: storage & spills
mine tailing piles
leaking pipelines
underground storage tanks
underground injection wells (industrial waste)
chemicals used at wood preservation facilities
fly ash from coal-fired power plants
asphalt production and equipment cleaning sites

(4) Agricultural sources:
air pollution
chemical spills
fertilizers
livestock waste storage facilities & land spreading
pesticides
underground storage tanks
poorly constructed or abandoned wells

(5) Individual and household sources:
air pollution
fertilizers
homes
cleaners
detergents
motor oil
paints
pesticides
septic systems
poorly constructed or abandoned wells
