Mrs Columbo actually had her own series for a very brief time. She may not actually count as mentioned and never seen...
"Frasier's" brother Niles is married to an heiress named "Maris" who is conveniently absent in every episode. Her absence makes Nile's pining over Daphne ever so pathetic...
"Charlie" from Charlie's Angels was only shot from behind a leather chair, or as a voice coming out of a speaker.
In an interesting twist, the lead character in "Remington Steele" was made up as a phony figurehead for the lady lead's detective agency. The story begins when a mysterious stranger (Pierce Brosnan) shows up and assumes the role of "Steele", a person the detective never meant anyone to meet.

Mrs Columbo had her own series in GB and it was shown on Sunday nights sometime in the mid 1980s.
Ugly Naked Guy was never seen on Friends, but then again it's happen a good job!!

The obvious examples have already been mentioned - Charlie from Charlie's Angels, and "'er indoors" from Minder.
I think Rumpole of the Bailey's wife, referred to as "She who must be obeyed", was also never seen on screen.

In the series - late 80's and early 90's, Inspector Morse with John Thaw, his sergeant's wife was always mentioned, but we have never seen her.

Mrs Colombo was seen. In fact, she had her own TV series for a season. Stared Kate Mulgrew (Voyager's Capt Janeway).
But she was never shown on an actual Colombo episode.
