In "Star Trek IV: The Voyage Home," when Uhura and Chekov were on the U.S.S. Enterprise (aircraft carrier docked at Alameda Naval Center) she called in and said "Scotty, we're ready for beam-out." That's the closest thing I can remember...

Good question...
Uhara beams up in Mirror, Mirror and ends up in the alternate universe. But it is Kirk who calls for the beam up.
So is also with the party in City on the Edge of Forever, but again Kirk calls for the beam up.
She is beamed down with Chapel in "Plato's Stepchildren", but the phrase is not said. You only see them arriving.
She operates a tranporter in ST3:Search for Spock.
I got it....
ST5: She does have Scotty beam her an Sulu off the battleship Enterprise. She is the one who made that call. But she did not say "the phrase".

Except for 'Plato's Stepchildren' I can't remember any other time she left the ship. I may be wrong, but I don't think she ever did. I'd watch 'em tonight again for you, but I left them at home, and home is about 7000 miles away.
However, if you're claiming that she DIDN'T say it, then it's incumbent on the other person to show when she did.

The exact words "Beam me up Scotty" were never said on the show by any charcacter.
