Dont worry about your creative ability, just write what you feel in your heart and If he really cares about you that much then he will love it just because of the thought you put into it..

Maybe you should read a couple of poems to get yourself inspired. Although, honestly, it will have more meaning if you write it yourself. He will love you even more for that! Besides, if you feel strong emotions inside, the words will come out! Maybe if you listen to a song while writing it could help you also. Sometimes when I write and the words don't really come into mind, I just listen to a certain melody and it helps... Good luck!
