1) Log in at http://en.wikipedia.org/wiki/Special:Userlogin
2) Upload the image at http://en.wikipedia.org/wiki/Special:Upload (make sure you add the correct copyright tag)
3) Edit the article and type [[ Image:Imagename.jpg| thumb| Caption ]] (change imagename.jpg for the images actual name and extension and change "caption" for a brief description of the image)

I'm not sure if you have to be signed in or not, but on the bottom left of the links (on left side of pages, under the Search box) is a section called Toolbox. There is a link on the bottom left side of page called Upload File. http://en.wikipedia.org/wiki/Special:Upload
Click the browse button on this page and find your image file.
Choose a destination filename (same filename is just fine - ie filename.jpg.
