My former job, required me to be a work-aholic.
1. You get sick all the time
2. Stress
3. Have very little ways of venting anger and stress
4. Miss out on a lot of things. Spending time with kids and friends
5. A lacking sense of accomplishment

relationships and marriage would potentially be difficult. I am a workaholic and married. We argue a lot about how much time I spend at the office. However, I make more money than my husband and I think that bothers him. But it goes back to what I said originally - relationships re difficult to maintain when you work a lot or are dedicated to something else.
Nothing in life can be successful without sacrificing something else. Look at it this way, if you have a great work ethic, you have the potential to be very successful at what ever you wan to do. Don't be afraid to give it all your effort.
Its true that working a lot can cause stress. But usually it is because the person does not want to work as much as they are and aren't organized enough to meet deadlines or take on significant workloads. Stress does cause illness. But if you like what you do and like working a lot for it, this shouldn't be an issue. Keep your focus on what you know is right for you. Best wishes.

If the person continues being a work-a-holic they will miss out on many of the simple pleasures that life has to offer... they will not know what they have missed until it is too late to find it.
Someone who realizes they are a work-a-holic and works to ammend their ways will have the rest of their life - how ever long they have to live to reap the benefits of a life well spent noticing the beauty of the world that isn't found while making money.

Fast food outlets programmed into the phone's speed dial, collapsing exhausted at the end of your "day" i.e. 20 hours, increased risk of falling asleep, behind the wheel of your car, and/or missing your train/bus ride home.
There's a photo of you on the family fridge so loved ones remember what you look like - if they still care...
Interesting point to consider: On a person's deathbed, NOONE has been heard to say "Gee I wish I spent more time at the office!"
