1) It depends on the program, but the only absolute requirement is an undergraduate degree in any major (and even that can be avoided in certain situations). You don't need an undergraduate business degree, and in fact the two overlap quite a bit.
2) Most likely, you will need work experience. Top MBA programs have an average student with 4-5 years of working experience. Easier programs usually require at least 2-3 years.
3) You need to take the GMAT, a standardized test that is similar to (but ironically, probably simpler than) the SAT.
That's about it for basic requirements.

Yes. In India you need a bachelor's degree to do an MBA. There are a few campuses that provide a composite 5 year program which gives you a BBA and an MBA. In the US however, you can do an MBA without a bachelor's degree, provided you have a professional degree instead (e.g. Chartered Accountant, Management Accountant, Hotel Management, Tourism etc.). MBA stands of Masters in /of Business Administration.
In most cases a Bachelor's degree is required.
In USA and in UK(London School of Economics), they were admitting any Bachelor degree holder. The program was structured and took 2 years.
The basic requirements had no meaning in USA, their system allows for taking prerequisites, the program may take that much longer, you may not get proper sequence of courses.
Again prerequisites will depend on the specialisation.

most of the MBA programs ask for bachelors degree but will never insist for bachelors degree in business. any degree with min 50% marks is what for IIM's insist for.besides IIM many top businee schools ask for only degree (10+2+3 years of study) in any feilds.

You don't have to have an undergraduate degree in business in order to get into MBA programs. I know people that have gone into MBA programs from all kinds of disciplines, including business, psychology and English.

No. In fact, many MBA programs prefer you don't have an undergrad degree in business, especially if it wasn't from their school - they want to mold you in their school's philosophy.
Check out Fortune Magazine and Princeton Review for a list of the top B-Schools. Each school has different minimum standards (GPA, resumes, references and GMAT scores).
You should aim for an accredited B-School. While unaccredited may be cheaper and they will tell you that accrediation doesn't matter, to many employers it does matter.
