Yes, tell her. If the roles were reversed, wouldn't you want to be told? There is the chance that he is not that way anymore and exclusively into only one woman now . .. but, if I was the woman I'd like to know so I can have an open conversation with my guy. Also, since you are neighbors, that is very awkward. How 'bout getting a note to her anonymously via her car or something?

No. I think you should mind your own business in this instance. You don't know the circumstances -- you could end up giving yourself more trouble for butting in.
Butt in if you hear him beating someone up -- not their sexual activity.
Keep in mind you have to live there.

are you sure its a gilrfriend? because most guys dont change sides like that girls do but not guys. so it might be wise to ask him if he is protecting her. this lets him know that you are worried about her and will take a step if he isnt.

YES! He is putting his GF at risk for STDs and thats why the HIV/AIDS rate is exploding, from guys on the DL. I dont care if he uses condoms or not, its risky and she needs to know about it.

Maybe she already knows...? I don't know... I don't think it's your business. However, if you KNOW that he has some horrible disease (like, for a fact) then, yes, I think you should tell her. Otherwise, don't worry about it.
