A complete answer to your question is not possible, as it is an entire subject called speleology.
Generally speaking caves are formed in soft rocks like limestones when they are leached out by action of water, particularly underground water. The caves in hard rocks are formed by mechanical removal of a block having structural weaknesses like faults, fractures, joints, cleavages etc. Other causes are already indicated by Jo.

many things can form a cave:
1) Water build up and find its way downward, after the water is gone the top parts were little tiny holes, got covered and now you got a cave.
2) Mining
3) Natural disasters like earth quake etc
