Let's organise a bit: Judders can happen because of several causes:
Somewhat clogged injector(s) or weak fuel pump. I once had a Ford that started that juttering business and it turned out to be fuel pump. I discovered it when it totally failed and had to be towed home.
A bad ingition wire or something in the distributor--a bad rotor is sufficient to cause this problem in some cases.
Probably the last think I would expect is a transmission problem, but it could be.

There could be a multitude of obvious problems but the 2 that come to mind are the throttle position sensor has failed and is not giving the proper info to the computer. The other is the O2 sensor. If it fails it will cause a lot of black smoke to come out the tailpipe. Check the TPS first.

if it judders all the time it could need tracking and balancing done,
if it only does it when you break the discs could be warped, if it's acting like its gonna run out of petrol it could be HT Leads or spark plugs... it could be many thing's.
Have someone drive it who knows a fair deal about car's and they could probably point you in the right direction.

either a sparkplug or HT lead faulty or a engine valve burnt out ,have compressions checked then plugs also you could have a brake stuck on, go round and feel the brake hubs after driving the car for about 10 minutes if one is very hot that one is

Is it manual or automatic gears? It could simply be that you have not selected a low enough gear or need to press the accelerator a little more.

if the car drives normalat speeds above 35mph it sounds like you are in the wrong gear dropdown a gear and try again

Sound like either your carborator or fuel injector. Not sure which one your car has. You might try putting a fuel injector cleaner in your tank when you fill up your car. Believe me you don't want to have trouble with your fuel system.
