Drug designers take a compound with known pharmacological activity, modify it somehow, and then test it. Its far too hard to predict the effect of chemical structures, so its really just trial and error. Usually they start with some compound that occurs in nature. For instance, procaine (Novocain) was developed by modifying cocaine. Antibiotics were all extracted from bacteria or fungi.
And to say we have no better medicines than 1901 is pretty absurd. I wouldn't even be here right now if it weren't for antibiotics. Also, our anti-tumor medications and anti-viral medications are better than ever. We didn't even have either back in 1901. That may not sound like a big deal, but talk to a cancer survivor, or someone with HIV and they'll feel differently. Also, back in 1901, the consensus on treating psychiatric disorders was to lock people up and put them in a straight jacket. If that didn't work, shock them or remove part of their brain. Anti-psychotics and SSRIs may have a lot of side effects, but you have to admit its an improvement over straight jackets and lobotomies.
The reason why we have more surgeries is because we've gotten better at that as well. In 1901, there was very little we could accomplish via surgery. Today, doctors can fix a brain aneurysm without even opening your skull. They have become so much less invasive that they can be performed on much older people without excessive risk.
I will agree, however, that some of the drugs prescribed today (like sleep aids) are very addicitive. But, uhm, in 1901 heroin was actually prescribed by doctors as a 'non-addictive' alternative to morphine. So I think its safe to say we've come a long way.

It doesn't actually cure anyone. Doctors assume so, but in all reality they are as advanced in medicines as they were in 1901.
Notice how many more surgeries? More than ever before!
How many more ADD/ADHD kids? More than ever!
How many sleep aid dependents? More than ever!
Does this ring a bell yet? That's why I don't go to the doctor because he'll be just as clueless as I am.
