Sure, there may be one that is better than others overall by a small margin, but it really depends on the management of any particular store. I worked in many fast food restaurants throughout high school and even two different McDonald's and they all had their own quirks.
If I did have to guess though, I would say a sandwhich shop like Subway, Firehouse, or Quiznos where the volume isn't so crazy. You'd have to figure 6 double Cheeseburgers( at a dollar a piece) will make more of a mess than 1 footlong sub(approx. $6)

This seems weird right now, since they're in the news for the sicknesses with the green onions, but.....
I heard on a news show that Taco Bell was the cleanest place to eat, and that they have the least bacteria...
It had to do with the fact that all of their food items are precooked, and there is never raw meat in the stores....

I have seen some pretty nasty looking fast food places -- of almost every chain. The only one I haven't seen at least one look looking bad at some point -- Chick-fil-a!
