His old girlfriend is going to be a best man? That's not the way it goes. I would say hellllllll no is your ex joining us at the alter. She wants to be a guest, fine-- but no way in hell she is standing up there with us..

I wouldn't handle that. You should be his best woman, not some old girlfriend.
I would be suspicious. You should confront him about his relationship with this lady.

Confront it, communicate my feelings, and go from there. He should know how you are feeling. If he doesnt know what he is doing wrong then he will never know how its making you feel.
