A fetish is a sexual attraction to, and in some cases a desire to achieve sexual gratification with, a specific object or type of object. Fetishes can be for specific body parts (feet being a common fetish), or sometimes for objects (shoes being another common fetish).

The word really describes a range of behavior, from mild to extreme. At the mild end you have people who are aroused by something not typically considered a sexual body part, such as the foot. At the extreme end, there are people who are unable to achieve sexual satisfaction without the fetish object.

You probably have a fetish if you find something sexually arousing that is not a primary sexual characteristic of your partner, and you incorporate this into your sexual activity. To use the common example of the foot fetish, it's probably not a fetish if you're having dinner at a fancy restaurant and you play "footsie" under the table. It probably is a fetish if you kiss or lick your partner's toes as a part of your regular sexual activity.

The only situation in which you might need help with a fetish (by which I assume you mean help getting rid of the fetish) is if it is interfering with your life in some way. Many fetishes are harmless, even beneficial to a relationship, and may come about through association.

For example, I have a mild foot fetish, acquired at least in part because my wife has always loved to get footrubs, especially after she gets out of the bath. So there I sit, rubbing the feet of a naked, pink, steamy woman, lying spread out on the bed in front of me, with her feet in my lap. I think it's safe to say that virtually ANY heterosexual man would get sexually aroused under similar circumstances. So these days, I always get a tingle when she takes her shoes off and stretches her feet out in front of me. Is that a fetish? I think so. Is it a problem? Not if you like getting footrubs. :-)

On the other hand, if you can only achieve satisfaction through a specific combination of objects, this could cause a problem if the combination is dangerous or difficult to acquire or assemble. For example, some men like to be strangled (partially) during arousal and orgasm, because cutting off the flow of blood to the brain is said to increase the intensity of orgasm. Obviously, there are serious risks to life and health from this kind of a fetish -- it only takes a few minutes without oxygen to start irreversible brain damage.

So that's the thing to consider when asking whether you need help with a fetish. Is your sexual behavior exposing you to danger or risk of disease, injury, or other trouble? If so, you should probably ask a therapist for help. Is your fetish just something that gives you pleasure and even provides an added measure of intimacy for you and your partner? Then there's no reason to seek help.
