Everyone is right here in that they are asking what particular religion you perfer.

It would be illegal to ask this question on an application unless the place of employment is a private business because it could be viewed as potential religious discrimination.

I personally would chose to put down "No Affiliation required".

Affiliation means to attach as a member, or branch. If you don't know, perhaps you are not a member of any church or you don't practice any particular religion.
Check with the camp, but it is probably something they like to have in your records in case something happens and they have to call in a clergy person, pastor, priest, etc.
Perhaps you can tell them you talk to God, not any reverend in particular. Maybe THEY won't understand that. But to be fair, it's most likely a routine crossing of the t's and dotting of i's.

affiliation means membership, but is not as strict. like maybe to be a member you have to go to meetings, but you are basically affiliated if you get the newsletter.

basically affiliated/affiliation usually means associated, involved, tied to in some way.

To be affiliated with something means you are associated with it. You have a connection to it.
This means, "What church do you belong to?"
