Teaching terminology: Concept vs. Theme vs. Topic??

Help me please.

I need to define these three terms. Then I have to classify different titles/groupings to fit into each of them (in social studies).

The one I am REALLY stuck on is which of the three "urbanization" would fit into.

It's definitely not a theme. I would say it's a concept. The topic could be "the negative effects of urbanization".
