My nepenthes have two 6 inch long pitchers, but one is beggining to die. Starting at the leaf atop the pitcher ( hood), the brown has reached halfway down the pitcher. This happened to the other 25 it HAD on it too... IT now has over 15 new leaves with many baby pitchers, and last time I had babies, they browned after they got hal and inch long. What am I doing wrong? It doesn't dry out, but it doesn't STAY soggy. Is it a water issue? I believe it is. Please let me know if this is over-under watering, wrong soils, etc. PLEASE HELP!!!

ALSO: Can leaves that had a pitcher ( but it died and I Cut it off) grow another pitcher from the same leaf?

You said that your pitchers were not forming properly. Nepenthes need high humidity if the pitchers are going to form properly and it looks like this is your problem. Nepenthes grow in trees in tropical forests so they like to be humid without standing in water. A tray of water next to the nepenthes sorts out this: water it as you normally do.You could also grow it through another plant as the humidity of the other plants leaves will give it better conditions. Also Nepenthes need a compost of 50% peat based 50% orchid bark (both easy to get at a garden centre). However you look like you got a large one which makes me think it may be the Lowland form so if you are growing it in a house then I must say that you should get a highland form which are easier to grow. - http://www.borneoexotics.com/ I would requimend you look up this site and they have more information there and you will beable to find out which type you have got.
