Why do some males have a protruding "Adam's apple" and others don't?

for the same reason that some men have larger "other appendages" than other men, that's just the way that they were born.

The scientific name is "larynx". Men have larger, and longer, vocal cords than women. The thyroid cartilage is what you see as an Adam's Apple; it is the outward protection for the vocal cords.
