There are a couple of things, medically, that can explain it. Atopic eczema is one chronic skin condition that causes skin discoloration that may be the culprit. Since you've had this issue for so long, it's probably hereditary or an allergy. I've heard that proper hydration, minimizing stress, and sleeping more can help, but I can't say how effective those things are.
For a quick cover-up, use a concealer that's one shade darker than your skin (not lighter), gently blot it under your eyes with the tip of your ring finger, then use a liquid foundation the same shade as your skin all over your face, and then follow with a pressed powder. Viola, no more dark circles.

Put cucumbers for about 10-15 min. before going to bed and you should see a difference the next morning. It works for me! Also, celebrities sometimes use Preparation H. They swear by it! Also, drink lots of water + eat well. They sell lotions, which can sometimes be expensive (like Avon, see link) to help reduce puffiness. I also use powder concealer around my eyes and that helps a lot. Good luck!
