Are you serious??? Pyrex doesn't break or shatter. I know, I've abused my Pyrex bakeware much worse than that. The worst being oven to freezer.
The Pyrex must have been defective. If you still have the dish, return it to a store for an exchange.
If you do not have the dish, contact Pyrex for a replacement. They will also be able to tell you if it was defective to begin with.
I have never, ever heard of Pyrex shattering. It's made for people like me who abuse their cookware.

This just happened to me two nights ago with a pyrex casserole lid! I was pissed! It wasn't even old. I guess the rule of thumb is not to run water over it when its that hot, four hundred degrees. It makes me wonder what the warranty on pyrex is!

Never run any kind of liquid especially hot or cold water on hot glassware. It will crack or shatter!!! Wait until the glass cools down!

I have done this and not had a problem. Your dish must have had some kind of crack or ding in it. That is the only time I have had that happen.

thermal shock fracture, the thing is hot and you are differentially rapidly cooling it, it cant handle it, it breaks.
