There's a huge difference between the poverty, famine, epidemic disease, and death occuring in third world countries, and the relatively wealthy poor people in this one.
There are assistance programs here, and people can work for a living. They may not be able to make big money, but even those with little or no education can find a job if they choose. Remember, we have millions of illegal immigrants coming into this country to fill the jobs nobody wants. Some of them even work two or three of those jobs, and do pretty well.
There are homeless here, true, and mental illness, which qualifies as disease, is a huge factor among them. Our system fails miserably to help them. Giving money to them would not improve their lot, however. Putting money into treatment and housing might.
But, to answer your question, Americans aren't dying by the hundreds of thousands due to their poverty. And, to a great degree, they have options that people in third world countries simply don't have. If I have to choose between helping someone who dying of poverty, and someone who is not, I prefer to help the ones with the most need.
But, we don't have to make that choice. We do both.

It depends on where it is. Sometimes like in the tsunami effort. Many sweat shops were wiped out. Us companies like the gap and nike "donated"millions to the getting the industrial secdtor up and running. I think there may have been another motive. Also it makes better PR.
As George CArlin said if you want to wipe out poverty find a way to make money out of helping the homeless.
The government is a tool of their bigest supporters....corporations.

Its not just the U.S. gov't. Ask Madonna and Angelina Jolie why they give money to African countries when there are plenty of starving people in our own country.
