The same thing happens to me sometimes. I know that in college, some professors offer students who have test anxiety (which is what you have) to take the test in a separate room with no one else in there. Many people do not do well because they feel pressured to finish when other students around them begin to get up and turn in their papers. If you feel that this is one of your problems then ask your teacher if you can take it in another room.
However, if that is not the case and like me you just start to panic, try this: Make sure you are studying and concentrating on things you aren't too sure about. Right before the test look over everything again, and again, concentrate on what you have the most trouble on. When the test comes, take a deep breath and close your eyes, try to ignore everyone around you. What they think does not make a difference, especially if you get an A and they don't. If you get stuck on a question, try to picture the page it was on in your head, kind of read it in your mind. I know that sounds foolish but it works for me. If you get stuck on a question, skip it and come back to it later, sometimes questions later in the test can help you with one you didn't get before. If they don't help and you still don't remember, don't sweat it, you did the best you could.
If none of this helps, talk to your teacher and see if the two of you can come up with something together. Most teachers are willing to help their students in any way that they can.

You should study a head of time. You should start studying for a test as soon as the teacher starts the new material. Don't leave it till the end because you will forget. You should also change your study habits. There are many ways to study so find your own style. Reading and rereading does not help all students.

It does sound like anxiety... you have to learn to relax and breathe.
Hopefully you are studying the right way too... if not ask your peers or a tutor to help you improve your style.

Here's a seemingly weird suggestion...
I read about this and I wish I would have known about it when I was in school... you know the herb rosemary? It is known as the "memory herb" in complimentary medicine or aromatherapy.
(You may want to find out if you have an allergy or a sensitivity to the herb first before you try this.)
Buy a bottle of rosemary oil/essence put a few drops on a facial tissue (Kleenex) and smell it while you study... When you go to write a test have a tissue with the scent on it, calm yourself, relax and take a whiff of the rosemary oil.
Smell is a strong memory trigger.

Study for the test in advance. Clear your doubts and revise the day before the test.
It could also be an anxiety disorder so check with a psychiatrist.

either ur studying to hard or studying to little

if u study to little u won't have enough time to remember everything
if u study to much- u just forget everything

u should like make studyguides for all the important stuff u need to know for a test that's what i do when i play for the school basketball team! it works

study for the test very well. I think you don't study very well. Thats why you forget everything during test. So one suggestion is- if you can't understand something, memorise the whole thing.

OK HERE'S WHAT YOU DO WHEN YOU STUDY DON'T EAT ANYTHING AFTER AND DON'T WATCH ANY T.V!!!

FIRST EAT SO YOU DON'T GET HUNGRY!!

SECOND STUDY REALLY HARD FOR AT LEAST 3 HOURS

THIRD GO STRAIGHT TO BED AND TAKE A SHOWER AFTER YOU EAT I FORGOT THAT ON!!!

P.S. WHEN YOU GO TO SLEEP NO T.V NO MUSIC NOTHING OR ELSE YOU'LL CONCENTRATE ON THAT THE WHOLE TIME AND YOU WON'T REMEMBER WHAT YOU STUDIED!!!

study harder. Don't just read over stuff, memorize it, burn it into your mind and check yourself by quizzing yourself before the test.
