Nothing to worry about. The only discomfort with those "horrid instruments" comes from the fact that they might be a little bit cold :)) The rest....you might see them as big and horrid but they are designed in such a way so they won't produce any harms...C'mon...it's for your health ;) Be a woman!

It's not bad at all. And the pain is equivalent to one sharp menstrual cramp. It goes away instantly. They make you lay on the exam table and if you're doc is good, she will use a plastic speculum (it holds your vagina open so the doc can see inside, just makes u feel a bit stretched) instead of a metal one. The metal one is often cold and must be cleaned after each patient. The plastic one is disposable and room temperature. The doc will look around in there, stick a few q-tips in to get a sample of the discharge and do the pap smear (the little pinch where she takes some cells from the uterus) and that's it! It takes less than 3 minutes usually. The doc may also put on a glove and put some fingers in there to feel if you have any lumps, growths, etc. That's more uncomfortable than the pain part. Every woman is nervous the 1st exam, but it's fine. The doc has probably had nervous patients before, so tell him/her that it's the 1st one for you. And don't embarrassed, just make sure you clean yourself very well the day of the exam. Don't worry about what it looks like to the doc. They've seen them all, believe me. You'll be fine. And don't cancel b/c it's extremely necessary for your health! Good luck!

Don't be scared! This can be difficult if you are nervous, but it gets easier all the time. The key is to relax when the speculum is inserted, if you tense up is can get uncomfortable. The thing is, it's over really fast if you relax, relax, relax your muscles. Also, some doctors will warm the speculum or even use a plastic one so it's not so much of a shock to your body. And as for the embarrassment, it's not so bad. The first time is always hardest, but just remember that they have done this many times before and what you have down there is what all women have down there. They also have different sizes of speculums, and will use the size they see fit. The main goal is not to hurt you, but to make sure you are healthy! Keep breathing and it will be over before you know it.
Just one thing, when they do the pap smear they will scrape you 3-4 times inside while the speculum is inside. This is probably the most uncomfortable part but VERY necessary to do. Like I said, it's over fast and it's usually the last thing they do before it's over. When the take the speculum out, they will usually feel the inside of your pelvis with the fingers, which does not hurt at all. Good luck and do not cancel!

Just let the doctor know you're nervous, they can explain exactly what they're doing.
Generally, you'll go into the exam room with all your clothes on. The doctor will come in and talk with you, find out health info, why you're there, etc. Then doc will leave and ask you to change into the johnnie, usually with the opening in the front so the doc can also do a breast exam. Cover your legs with the sheet they give you. The doc will come back in after knocking, and will ask you to lay down on the table and scoot forward, so you can put your feet in the stirrups, so your legs can open wide enough for the doctor to have access. The doc might start with this or with the breast exam. Then the doctor will put some lubricant on the speculum(just a device the kind of holds you open for the doctor to see inside), and will use basically a q-tip to get a swab rom inside you(a pap smear). Then the doc might remove the speculum and insert a finger while pressing down on your abdomen, and might also put a finger into your anus to make sure everything is ok. Then the doc will probably leave while you get dressed, come back in and talk about any questions you might have, etc.
I know it sounds scary, but it's great that you're taking good care of yourself. Don't worry too much, once you go through it the firts time it's a piece of cake because you'll know what to expect.

Moxie1313 gave you the perfect description of what to expect. I don't think that she left out anything. Read what she wrote to you and you will know what to expect.
If a pelvic exam hurts it is usually because you are scared and getting all tensed up. Relax! If the doctor is any good at all she will know that this is your first exam and that you are going to be scared and nervous. She should take the time to go through each and every thing that she is doing so that you know what to expect. BTW, most of the instruments that used to be made out of ice cold metal are now made out of disposable plastic and not cold and awful like they used to be. It is not nearly as bad as you are thinking it will be.
I can't help but wonder why you are having the exam if you are so scared and do not want it. Is it because you have requested birth control? If you are having sex how could you be afraid of an exam? Sex is much more intrusive! Is someone forcing you to have the exam? I would wonder why.
Other than that, the other replies have answered your questions.
Also, think about something else and it is really over before you know it. If the doctor is any good she will be done before you know it.

Just relax, no one wants to have a pelvic exam. But you can't cancel its one way to make sure that everything is okay. If you have a good doctor they should walk you through every step that they do, if you really get worked up about it you will probably make yourself sick. If you are worried about pain maybe take some Motrin before hand. Most people don't have pain and if there is some it's usually just some cramping for a little while afterwards. It will be okay. If you want call the nurse and she can go into details on what will happen, but it will always sound worse then it really is.

You've already gotten some awesome feedback, but just to add... it's going to be uncomfortable and you're not going to "like" it. Just remember to relax and try to think about something else. Make sure to tell your physician it's your first pelvic exam as well, and they will be more gentle with you. Remember they've seen THOUSANDS of vaginas and yours will be NO different from any of the others they've seen. My mom and a good friend told me a couple stories about down there visits and they put a smile on my face while I was having my first exam, maybe they'll do the same for you!!
-- A woman is sitting at the OB's office and asks to give a urine sample. She realizes after "going" there was no more toilet paper in the restroom. She quickly grabbed a tissue from her purse and proceded to the exam room. While on the table her doctor started to chuckle... (could you imagine??) he called in one of the nurses to come take a look. My friend asked what the matter was. The doctor asked if she planned on going somewhere soon on vacation. She said, "No, why?" He pulled out a postage stamp from between her legs!!! It was stuck to the tissue she used in the restroom... hahahaha... too funny!
-- Another "friend" was getting ready to go to her OB appointment. She was taking a shower before she left and realized she didn't have a clean wash cloth. She picked up one of her daughters and used it and then went to her appointment. When laying on the table her doctor commented, "Wow, you really cleaned up well for this..." The friend asked what he meant and he showed her... apparently her daughter used glitter body wash so she was "sparkling clean" down there for the doctor! TRUE STORY!!
Good luck at your appointment! Don't cancel. Just get it over with!

Yeah..it's uncomfortable, but your health is more important. Just keep in mind that you're not the only one going through it, and your doctor has seen this day in, and day out.
Be open with your nurse and doctor...tell them you're uncomfortable, and if they have any bedside manner whatsoever, they'll sympathize. (Also, if they don't do it already, request that they warm up the speculum for you...nothing like a cold one for a wake up call).
The good thing is, they don't take long, and personally, aside from the discomfort and the fact that a stranger is looking at your woo hoo, it's not really as bad as you think.
You can never be too careful with your health.

i had my first one when I was 14..I threw a fit in the Dr's office, and my mom had to force me to have it done.. strap me to the bed, remove my clothing and so on and so forth.. I threw that fit for no reason... it was not that bad really. It was a little embarrasing, and a little discomfort, but not horrifically painful or anything. The tools they use, are not that bad really, the worst part is when they stick the q-tip in for the swab... honestly.
I felt like a fool when i left there, because of the fit that I threw.
Just relax, and don't worry, besides it is good for you to have it done. better knowing that all is ok, than knowing that all is not ok.

Throw a fit!!! REBEL!!! Don't give into conformity!!! We need people who aren't afraid to stand up for what they believe in!!! If you don't want to get a pelvic exam, ask to get a hysterectomy or SRS so that you don't have to go there again. There are options for those who hate being girls but still want to be girls. (I know it's confusing.)
