Wearing you heart on you sleeve means that you are very open, unguarded and caring. It's the exact opposite of being made of stone.
It suggests that one is sensitive. One who wears their heart on their sleeve doesn't hide their emotions and is honest which can leave them vulnerable.

The term is not particularly about being in love. The term comes from the visual: Ones heart (true feelings) are out in the open for all to see, not hidden as they may be if one were trying to decieve or had an agenda. Also because the heart is on the outside, it can be hurt. Overall it's considered a good trait.

Wearing your heart on your sleeve basically means you let all of your emotions hang out in the open. Some women/girls are very sensitive and emotional when it comes to guys, so once the guy hurts them, they're completely heartbroken. You could also think of it this way: Your heart is something very precious, therefore you should always keep it hidden and safe until you find something or someone worthy of your love. However, if you choose to just let it "hang on your sleeve" you're exposing it to everyone and by doing so, you make it easy for people to hurt you and break you heart.

Being obvious (usually by talking about it, even telling the world about it) that she's in love, especially if the guy doesn't know about it yet. He can wear his heart on his sleeve too.
