YES, in College, and k-12 all they do is go to the registrar at a college and fill out ao form or they simply go to the guidance counselor at a k-12 school and the change takes place. I had to do this a few weeks ago at a Junior High, I mistakenly gave another student a high test score and shorted another. We are human too!

Yes,

No grade is final until they send the report card. Teachers turn in grades all the time and they may discover an error, then they correct it.

It depends on the school or district. There are different rules and procedures for everything. If a teacher finds out a student cheated they have the right to go back and change the grade.

College-I don't know. K-12, yes. Sometimes errors are made in recording the grade and are subsequently corrected.
