Normally, I click tools, internet options, and then delete cookies, all files (including offline), and then clear history. Today, while looking at the "history," after deleting all files and clearing history, that although zero files show up, if I click "view" "by most visited," it still displays a bunch of sites, and some of which I havent visited for months. How do I completely clear all files?

I tried everything you suggested below. IT'S STILL THERE! Everything is deleted except for the "most visited," and they're not even sites I've visited more than once! I can't believe that not a single one of you knows how to fix this!

ok, if clearing the history in internet explorer doesn't work, here's how.

click "Start" > "Run" > type "%userprofile%\local settings".

go into the "History" folder. and press "Ctrl+A" to select all the days/weeks/months... of history, press "Delete" > click "Ok".
All your history should be deleted.

Now, if you also want to clear out your temporary internet files (they can enable people to see what sites you have visited)
here's how.

(Repeating some of the steps as above)
click "Start" > "Run" > type "%userprofile%\local settings"
go into the "Temporary Internet Files" folder.
and press "Ctrl+A" to select all the files...
, press "Delete" > click "Ok".
All your Temporary Internet Files should be deleted.

You can also do somewhat the same if you are ultra-paranoid by repeating another process similar to the previous ones, except going into the "Temp" folder and clearing out those files as well.

hope that brings peace to your worried mind :D

Don't know which version of Windows you are using, but with 98, I bring up Windows Explorer, go down to the Windows folder, click on it to open, then go to Recent, click to open folder then delete all the files in that folder, then go to Temp, again click on that folder (it's going to have a ton of folders) and delete everything in it, then go to Temporary Internet Files, again delete everything.

Plan on spending a day to do it.
