Straight Zinc Oxide.. It stays on no matter what but I will warn you its horrible to get off when you want to wash it off.. lol. You can buy this any where its pretty cheap like a buck for a tube. I use to get it at CVS in the first aid aisle.

A & D ointment it smells really bad but i swear it the best stuff ever its think but very creamy its so good its actually kinda hard to wipe off and i swear with my kid if she had a diaper rash at 7 AM it will be gone by 2pm if not earlier. plus i used to get really bad kidney and bladder infections when i was a kid so i was raw ALL the time to the point that the DR said i wasnt allowed to wear pants only dresses and cotten panties and my mama used that stuff on me not only does it stay on but it also sooths the pain so i can not only vouch for my baby but also personal experience

Aquaphore, its expensive but great diaper ointment and a little goes a long way..my sons rash disappears within a few hours after using this stuff. Plus you can use on scrapes and cuts too!

I think if you need something that is great on Dipper rash, if your child gets it all the time. You can try destine first but if you are like me my daughter got so bad Dipper rash( because she pooped all the time like 10X's a day) ( she was early and needed a special formula and we were in the stages of finding out what was wrong.) I had to have my pediatrician prescribe something and my daughter bleeding Dipper rash went away like in a few days and I kept that on her like every time I changed her till she was pooping normal and then used destine and I never had any problems again. I think that if it's a reoccurring thing that you can not get rid of then ask your pedi. for a perscription ointment.

Desitin (the original stuff, not the creamy one), Aquaphor and Boudreaux's Butt Paste are all heavy duty diaper rash stuff that will clear it up AND stay on. The only thing i would caution with Boudreaux's Butt Paste is that it stains clothes.

Aquaphor is a great investment since you can use it on scrapes and burns and very dry skin.

This will sound weird, but....go to Walmart or the equivalent. Go to the pet products aisle, and find "Bag Balm". It is in a green square tin. This is the BEST stuff for diaper rash that I've ever seen! It clears it up within 1 day, and is really thick without being gross. I wouldn't put anything else on mine! (Babies, that is, not my own rear end, lol.)

Of course you can use powder. It's talc that is the issue. There are talc free powders that you can use that are safe like Burt's Bees, Crabtree and Evelyn, Belli Baby, etc. Powders can be useful if you live in a moist climate or if you have a baby who tends to be sweaty. Ointment and powder have different jobs: creamy products moisturize and provide a physical barrier from irritants, powders absorb moisture and wick it away from the skin.

I used straight zinc-oxide ointment for the fist year and never had diaper rash. Also I changed my daughter's diaper constantly so she wasn't sitting in a dirty diaper for very long.

You need to get this stuff call Butt Paste. No it is not a joke. It is the best stuff on earth. It is pretty thick and stays on really well. I think it's by a company named Bourdeux or something like that. You can get it at any local pharmacy or drug store.
