Check out the articles on SitePoint.com I've been visiting this site for a few years and rarely have to go anywhere else, the forums are excellent.

They also sell great books:
"The Javascript Anthology" http://www.sitepoint.com/books/jsant1/
"The PHP Anthology" http://www.sitepoint.com/books/phpant1/

Well HTML is usually used to build the forum. JavaScript can be used to check the responses (make sure there is an email address in the email address field, etc) and then there is a server side process usually to process the form.

There are scripts at http://javascript.internet.com/ and there are also some scripts at http://archive.devx.com/sourcebank/

But I would not really rely on JavaScript since it is client side and can have different (or even no) effects on the client side

They are probably referring to mortal sins, Honestly, I'm not believing in God this week. Maybe next week a revelation will occur and I'll change my mind. Don't sweat it, the way I see it, If I live a good life and treat others the way I want to be treated, and then can't get into heaven, - well it makes me wonder if that's where I want to spend eternity. I had a person tell me all those "Chinese who don't know about Jesus are going to go to hell" - I told her if I would rather go to hell with them than spend forever with her. If god loves us, why should we be going around living in fear of being denied eternal happiness. If Doubting Thomas had not gotten to stick his fingers in Jesus' wounds, do you think when he got to the pearly gates God would say "Sorry Tom" - the whole thing is ridiculous and most likely taken out of context. If you need to believe, try another church.
