Depends on the country. If you are in Japan, you can order through the Foreign Buyer's Club's Express service (I think their website is fbcusa -- you can see if they serve other countries). Also, a friend recommended ExPat Express. Both have websites for more info.

Maybe the best way is to contact the US Embassy in your country. (-: They are there to bail citizens out of jail, but they are also there to promote trade, and may be able to direct you to the right people.

Warning: some countries have marketing agreements with suppliers in other countries, and won't send stuff to those countries. I'm thinking Musician's Friend, which won't take you order if even your credit card billing address is in Japan. I'm not sure what Gap's policies are. I do know they have a lot of stores in Japan, but not in my neighborhood.
