Actually, they just leave the court and go to the locker room. Normally before game and half time they take care of nature calls. But if sick or it becomes necessary, they just come out of the game and go to the locker room. TV timeouts and team timeouts help. Basketball players have numerous opportunities to relieve themselves so it doesn't become an on court situation.

I have often thought that injuries that send a person briefly to the restroom was really a call to nature. I have wondered about this very thing. I still feel when a football player holds his gut and then goes off the field briefly, it is to take a whizz. Same with basketball players. Hocky players probably just piss and keep on playing.
