You're not tucking your head down far enough below your arms when you hit the water. When done correctly, the water will come around the sides of your face, rather than splash down on the top of your goggles.

Try tightening the straps and use double strapped goggles so one strap is placed higher than the other. That will help hold the goggles in place. I also use to do a 'good luck push' and push on the goggle so they suctioned to my eyes.

You need to buy a pair that fit your properly. This is a trial and error process. I bought a pair that don't work in the pool, when I push off the wall they leak a little, but they are good for open water swimming. I bought a new pair of the old ones I used and have never had a problem in the pool.

Not tight enough. If tightening them bothers your eyes, try wearing a watermask instead of goggles.

If you are referring to swimming goggles, they have to be too loose. Use wider elastic bands. tuck them underneath swim cap put them on when on dry land.

You may need a bigger pair if the strap is coming undone. Or you may need a smaller pair or to tighten them if they just come off when you dive, etc.
